Pacific Hospice Care’s team includes professionals, paraprofessionals and volunteers who are sensitive and responsive to individual patient and family needs:

Our Personalized Care Team includes:

  • Nurses who are skilled in providing comprehensive assessments and symptom management. Our nurses are trained clinicians who provide hands-on care, education and support about the patient's illness and what to expect.
  • Social Workers who are available as friendly and skilled listeners to provide emotional support and help with problem solving, financial issues and planning.
  • Chaplains who offer spiritual support, working with the family’s clergy when available.
  • Hospice Volunteers who are specially trained and are an important element of the hospice team; being available to spend time with patients when they are needed most.
  • Musical Therapists who provide alternative support and comfort through the therapeutic sounds of music.
  • Hospice Physicians who work together with the patient’s attending doctor and are available for pain and symptoms management consultation as well as to make house calls as needed.
  • Home Health Aides who are available to help with personal care, such as bathing or dental and mouth care, and can help with light cleaning, housekeeping, light laundry, heating meals and short errands.
  • Bereavement and Spiritual Counselors who provide grief and loss counseling, memorial services and other support for family and loved ones.
